PARENTS AND ATHLETES PLEASE READ THIS INFORMATION SO YOU KNOW

WHAT TO DO WITH THIS FOLDER!

1. You must include WIAA physical card signed by both the doctor and

parent/guardian. If this is the athletes’ second year, the alternate card

needs to be included with the physicians WIAA physical card. Your child’s physical exam is

valid for two years, if the physical was taken during the summer.

2. Completely fill out the emergency card. Be sure to include a

parent/guardian signature.

3. Pull off the sheet on back of the Athletic Code, sign, and put sheet back in

the folder. Keep the Athletic Code for your information.

4. Athletes will need to pay a $30.00 user fee for each sport he/she

participates in. Make checks payable to Horace Mann Middle School.
